At approximately 0845 hours on 12 July 1968 at Strike Pad located at LZ Sally, WO Narburgh, aircraft commander of UH-1H 66-1118, had landed and picked up five Vietnamese and two American passengers to relocate for the supported unit. WO Narburgh was at the controls of the aircraft and attempted to pick up to a hover. Upon doing so, the aircraft tipped to the right. The aircraft commander immediately lowered pitch. The aircraft hit heavily on the right side, resulting in the right cross tube being damaged. Inspection of the aircraft revealed that the skid shoe had wedged in the PSP, preventing a normal takeoff
