Accident Summary:
ON 21 JANUARY 1971, THE CREW OF UH-1C, SN 66-591, CALL SIGN FALCON 82, WERE RETURNING TO DONG TAM ARMY AIRFIELD UPON COMPLETION OF A TACTICAL MISSION. THEY HAD FLOWN APPROXIMATELY SIX HOURS THAT DAY. THE AIRCRAFT LANDED AT DONG TAM, PROCEEDED TO THE REARMING POINT AND THEN TO POL. UPON REFUELING, THE AIRCRAFT COMMANDER REQUESTED TO CLOSE LEFT TRAFFIC FOR HIS UNITS REVETMENTS. THE WINDS WERE APPROXIMATELY 090-DEGREES AT 10 KNOTS. THE AIRCRAFT COMMANDER WAS AT THE CONTROLS OF THE AIRCRAFT AS THEY HOVERED TO THE ACTIVE, PERFORMED A GOOD HOVER CHECK, AND PROCEEDED TO TAKE OFF. THE AIRCRAFT WAS CLIMBING OUT AT ABOUT 40 KNOTS AND 25 FEET WHEN THE NOSE OF THE AIRCRAFT YAWED TO THE LEFT AND THE RPM STARTED TO DROP. ANALYZING THIS AS AN ENGINE FAILURE, THE AIRCRAFT COMMANDER LOWERED THE COLLECTIVE SLIGHTLY TO REGAIN ROTOR RPM. SIMULTANEOUSLY, HE ATTEMPTED TO FLARE THE AIRCRAFT TO DISSIPATE AIRSPEED. THE SKIDS LIGHTLY HIT THE DIRT EMBANKMENT AROUND THE PERIMETER AS THE AIRCRAFT SETTLED FORWARD. THE AIRCRAFT THENN STRUCK AND WENT THROUGH ONE BARBED WIRE FENCE; IT THEN HIT AND WAS SNAGGED ON A SECOND BARBED WIRE FENCE. THIS CAUSED THE AIRCRAFT TO FLIP UP NOSE LOW AND IT CAME TO REST ON ITS LEFT SIDE. THE CREW MEMBERS EXITED THE AIRCRAFT WITH LITTLE DIFFICULTY AND WERE PICKED UP IMMEDIATELY BY ANOTHER AIRCRAFT WHICH WAS DIRECTED BY DONG TAM TOWER. THERE WERE NO MINES IN THIS PARTICULAR PORTION OF THE PERIMETER. THERE WERE, HOWEVER, SEVERAL TRIP FLARES RIGGED TO THE BARBED WIRE. THERE WERE AT LEAST TWO IGNITED BY THE CRASH AND THEY ARE THE PROBABLE CAUSE OF THE ENSUING FIRE. AT LEAST ONE FLARE WAS BURNING NEXT TO WHERE FUEL WAS SPILLING FROM THE RUPTURED FUEL CELL. APPROXIMATELY THREE MINUTES AFTER THE CREW WAS EVACUATED, THE AIRCRAFT EXPLODED FROM THE FIRE AND ORDNANCE ON BOARD.\\
