Accident Summary:

 THE AIRCRAFT WAS FLYING A LOGISTICAL SUPPORT MISSION TO RESUPPLY AN INFANTRY ELEMENT IN THE FIELD. IT WAS LOADED WITH TWO PASSENGERS AND APPROXIMATELY 600 POUNDS OF CARGO AT FIRE SUPPORT BASE (FSB) ^TOMAHAWK.^ THE HOVER CHECK PERFORMED PRIOR TO TAKEOFF REQUIRED 35 PSI TO 39 PSI TORGUE. THE AIRCRAFT LANDED IN A HASTY LZ UNDER DIRECTION OF A GROUND GUIDE, AND VERBAL CLEARANCES FROM THE CREWCHIEF AND RIGHT DOOR GUNNER. THE ELEVATION WAS APPROXIMATELY 2200 FEET, WINDS WERE CALM, AND THE OUTSIDE AIR TEMPERATURE WAS ESTIMATED AT 29C. THE AIRCRAFT TOUCHED DOWN ON A SLOPE TO THE RIGHT AND TO THE FRONT. THE AIRCRAFT WAS UNLOADED AND RELOADED WITH TWO PASSENGERS AND APPROXIMATELY 100 TO 150 POUNDS OFCARGO. THE GUNNER AND CREWCHIEF EACH CLEARED HIS RESPECTIVE SIDE OF THE AIRCRAFT FOR TAKEOFF. AS THEAIRCRAFT COMMANDER BROUGHT THE AIRCRAFT TO A HOVER, THE AIRCRAFT SLID RIGHT, AND TILTED TO THE RIGHT AT AN EXCESSIVE ANGLE. WHEN THE AIRCRAFT COMMANDER RECOGNIZED THIS SITUATION, HE BEGAN REDUCING COLLECTIVE PITCH AND APPLYING LEFT CYCLIC PITCH. AT THIS TIME THE RIGHT SKID BROKE LOOSE FROM A STUMP, AND BEFORE THE LEFT SKID CONTACTED THE GROUND, THEAIRCRAFT BECAME AIRBORNE. THE AIRCRAFT RETURNED TO FSB TOMAHAWK WHERE IT TERMINATED AT A HOVER. THE CREWCHIEF AND THE GUNNER GOT OUT, SAW THE DAMAGE, AND ADVISED THE AIRCRAFT COMMANDER TO RETURN TO THE UNIT MAINTENANCE AREA. THIS WAS ACCOMPLISHED AFTER UNLOADING THE PASSENGERS AND CARGO.\\
