Accident Summary:

 ON THE MORNING OF 22 SEPTEMBER 1969, 1LT BYSTRAK WAS SCHEDULED FOR A 90 DAY STANDARDIZATION CHECK RIDE WITH A COMPANY INSTRUCTOR PILOT WO1 DELLAZOPPA. AFTER PREFLIGHTING UH-1H 67-17702, THE CREW, 1LT BYSTRAK FLYING IN THE RIGHT SEAT, WO1 DELLAZOPPA FLYING THE LEFT SEAT AND THE CREW CHIEF AND GUNNER PROCEEDED TO THE TRAINING AREA NEAR LZ SALLY. AFTER LANDING, THE CREW CHIEF AND GUNNER DISMOUNTED, REMOVED THE MACHINE GUNS AND AMMUNITION FROM THE AIRCRAFT AND WENT TO THE SOUTH SIDE OF THE TRAINING AREA TO ACT AS SECURITY. AFTER COMPLETING SEVERAL MANEUVERS WO1 DELLAZOPPA THEN BEGAN DEMONSTRATING THE EMERGENCY PROCEDURES FOR TAIL ROTOR FAILURE BY HOLDING LEFT PEDAL AND CONTROLLING THE YAW OF THE AIRCRAFT ON FINAL WITH THROTTLE AND COLLECTIVE (TORQUE). THE APPROACH WAS STARTED WITH BETWEEN A NORMAL AND SHALLOW SIGHT PICTURE. AS HE NEARED THE GROUND WO1 DELLEZOPPA NOTED HE WAS SHOOTING SHORT IN THE VICINITY OF SOME GRAVES ON THE WEST EDGE OF THE FIELD. TO CORRECT FOR THIS HE EXTENDED THE FLIGHT PATH AND THUS SHALLOWED OUT THE APPROACH ANGLLE. ON FINAL APPROACH OVER THE GRAVE AREA THE RPM WAS BEING MAINTAINED BY MANUAL THROTTLE CONTROL AND WAS DOWN TO 5900. THE RIGHT SKID TOUCHED DOWN WHILE THE AIRCRAFT WAS IN A LEFT CRAB AND THEN SKIPPED FURTHER TOUCHDING DOWN AGAIN. ON THE THIRD TOUCHDOWN THE SKID HIT A SMALL DEPRESSION AND DUG IN WHILE THE AIRCRAFT WAS IN AN APPROXIMATE 30 DEG CRAB TO THE LEFT. THE AIRCRAFT THEN STARTED TIPPING TO THE RIGHT AND THE IP ATTEMPTED TO ADD POWER AND PITCH AND TO STRAIGHTEN THE AIRCRAFT WITH RIGHT PEDAL. THE RPM AT THIS TIME WAS UP TO ABOUT 6200 AS HE WAS ADDING FULL THROTTLE. THE AIRCRAFT CONTINUED ROLLING AND THE MAIN ROTOR STRUCK THE GROUND. THE AIRCRAFT THEN CAME TO REST INVERTED. WO1 DELLAZOPPA WAS ABLE TO FREE HIMSELF FROM THE AIRCRAFT AND IMMEDIATELY BEGAN TO TRY TO EXTRACT 1LT BYSTRAK. THE PILOT OF A LOH (CPT GOODSPEED) THAT HAD BEEN USING THE TRAINING AREA ALSO, CAME OVER AND HELPED FREE 1LT BYSTRAK. THE CREW CHEF AND GUNNER ARRIVED AND ASSISTED IN THE OPERATION. THE CREW CHIEF REMOVED A SIDE PANEL AND PULLED THE QUICK DISCONNECT FUEL LINE TO SHUT OFF THE ENGINE. THE IP AND PILOT WERE TAKEN TO MEDICAL FACILITIES AT LZ SALLY AND TREATED FOR THEIR INJURIES.\\
